Lower Eyelid Blepharoplasty in London
Under-eye bags corrected with anatomically precise surgery. Every case is assessed, planned, and performed personally by Mr Ahmad Aziz, Consultant Oculoplastic Surgeon.
Quick Answer
Lower eyelid blepharoplasty removes or repositions herniated fat, and where needed a small amount of excess skin, through an incision hidden inside the lower lid or just beneath the lashes. It’s performed under local anaesthetic as a day case, with most patients back at work within seven to ten days. Early results show within two weeks, and settle fully by three months. Mr Ahmad Aziz, Consultant Oculoplastic Surgeon, performs every case personally.

Lower eyelid blepharoplasty is a surgical procedure that corrects persistent under-eye bags by treating the fat pads that push forward beneath the eye as the supporting tissue around them weakens with age. It is a different operation from upper eyelid surgery, and not everyone who notices puffiness beneath their eyes needs it.
The bulge under the eye is nearly always fat, not skin. That’s why creams, sleep, and hydration rarely make a lasting difference: surgery removes or repositions the fat directly, addressing the physical cause rather than covering it. Some patients are dealing with something else entirely. Hollowing, fluid retention, and pigmentation can look similar to true bags, and each needs a different treatment.
Every case at Eyes Defined is assessed, planned, and performed personally by Mr Ahmad Aziz, Consultant Ophthalmologist and Oculoplastic Surgeon, at King Edward VII’s Hospital in Marylebone. It’s one part of his wider eyelid surgery practice.
Who is a good candidate?
Not everyone who notices under-eye bags needs surgery. Some people have a hollow tear trough, fluid retention, or skin pigmentation rather than true fat prominence, and each of those responds to a different treatment. Our guide to 7 Natural Remedies for Under-Eye Aging covers some of the non-surgical options, and our piece on the differential causes of under-eye swelling explains how to tell a true fat bag apart from festoons or hollowing. A proper assessment is the only reliable way to know which applies to you.
You may be a candidate if…
- Under-eye bags present even after a full night's sleep
- You have a visible bulge that casts its own shadow, regardless of lighting
- You have bags that have gradually worsened over several years
- See little to no improvement from tear trough filler
- You're in good general health and realistic expectations for the outcome
What results can you expect?
1
Reduced under-eye bags
The herniated fat causing the bulge is removed or repositioned, softening the contour where the lower lid meets the cheek.
2
A natural, rested appearance
The goal is a version of you that looks less tired, not a different-looking eye.
3
Long-lasting correction
Fat pads that are removed or repositioned rarely return, so results hold for years, with proper aftercare.

How lower eyelid blepharoplasty works.
1
Preparation
Pre-operative instructions, including stopping blood-thinning medication in advance.
2
Anaesthesia
Local anaesthesia with sedation in most cases; general anaesthesia is available.
3
The Procedure
Fat is removed or repositioned via a transconjunctival or subciliary approach, typically in 45–90 minutes.
4
Completion
A brief recovery on-site, then home the same day.
Recovery and aftercare.
Most of the visible healing happens within the first two weeks, though the final contour continues to settle over several months. Clear aftercare instructions and follow-up appointments are provided throughout.
Days 1–3
Swelling and bruising peak; cold compresses and head elevation help.
Week 1–2
Bruising fades and most patients return to desk-based work.
Week 2–4
Makeup can typically be worn again; light exercise gradually resumes.
Month 3+
Residual swelling settles and the final contour becomes clear.
Risks and safety.
All surgery carries risk, and this procedure is no exception. These are discussed openly at consultation, before any decision is made.
- Bruising and swelling — expected in the first two weeks; resolves with normal healing.
- Dry or irritated eyes — usually temporary; more likely in patients with pre-existing dry eye.
- Ectropion — outward turning of the lower lid; risk is reduced with careful technique and assessment.
- Asymmetry — minor differences can occur; most settle as healing completes.
- Infection — uncommon; managed with antibiotic cover where needed.
- Changes in sensation — temporary numbness near the incision, usually resolving over several weeks.
The single most effective way to reduce these risks is choosing a surgeon with oculoplastic training who assesses ocular health before recommending surgery. The NHS overview of blepharoplasty covers these risks independently, and our guide on how to avoid blepharoplasty regret goes into more detail on choosing the right surgeon.

Frequently asked questions.
What is the difference between lower blepharoplasty and tear trough filler?
How long do the results of lower blepharoplasty last?
The fat that Mr Aziz removes or repositions rarely returns, so results are long-lasting for most patients. Natural ageing continues, but the underlying correction holds.
Is lower blepharoplasty painful?
Will I be left with a visible scar?
Often not. A transconjunctival approach leaves no external scar. Where skin needs to be removed, the incision sits just below the lash line and typically fades to a fine line.
